3 Bestimmungsgemäße Verwendung

3.1 Verwendungszweck

Die Orthese ist ausschließlich zur orthetischen Versorgung der unteren Extremität einzusetzen

und ausschließlich für den Kontakt mit intakter Haut bestimmt.

Ottobock empfiehlt, das Produkt nicht direkt auf der Haut und mit einem Strumpf zu tragen. Zum

Unterziehen eignet sich ein dünner Strumpf aus einem waschbaren, Feuchtigkeit absorbierenden

Material. Achten Sie auf einen faltenfreien Sitz des Strumpfs, da es sonst zu Druckstellen kom

men kann.

Die Orthese muss indikationsgerecht eingesetzt werden.

Der Arzt entscheidet über die Tragedauer des Produkts.

•28U25 WalkOn Reaction Junior: Die Orthese darf mit anderen dynamischen Fuß/Unter

schenkelorthesen kombiniert werden, wenn die biomechanischen Anforderungen dadurch

besser erfüllt werden können.

3.2 Indikationen

• Leichte Beeinträchtigung der fußsenkenden Muskulatur

• Leichte Beeinträchtigung der Knieextension

• Fußheberschwäche, ohne oder mit leichter bis moderater Spastik

z. B. ausgelöst durch

• Schlaganfall

• Traumatische Gehirnverletzungen

• Multiple Sklerose

• Neurale Muskelathrophie

• Peronaeuslähmung

• Infantile Cerebralparese (ICP)

• Beeinträchtigung der fußsenkenden Muskulatur

• Beeinträchtigung der Knieextension

Die Indikation wird vom Arzt gestellt.

3.3 Kontraindikationen

3.3.1 Absolute Kontraindikationen

• Moderate bis schwere Spastik im Unterschenkel

• Moderates bis schweres Ödem

• Moderate bis schwere Fußdeformitäten

• Beingeschwüre

3.3.2 Relative Kontraindikationen

Bei nachfolgenden Indikationen ist eine Rücksprache mit dem Arzt erforderlich: Hauterkrankun

gen/-verletzungen, Entzündungen, aufgeworfene Narben mit Schwellung, Rötung und Überwär

mung im versorgten Körperabschnitt; Lymphabfluss-Störungen – auch unklare Weichteilschwel

lungen körperfern des Hilfsmittels; Empfindungs- und Durchblutungsstörungen im Bereich des

Unterschenkels und des Fußes.

3.4 Lebensdauer

Das Produkt ist bei bestimmungsgemäßer Verwendung ausgelegt für:

Junior 1Jahr (Schuhgrößen EU 24-27, 27-30, 30-33, 33-36)

Erwachsene 2Jahre (Schuhgrößen EU 36-39, 39-42, 42-45, 45-48)

Wird das Ende der Lebensdauer erreicht, das Produkt durch Fachpersonal auf Funktionstaug

lichkeit überprüfen lassen. Ein Gebrauch über die angegebene Lebensdauer hinaus geschieht

auf eigene Verantwortung.

3.5 Nutzungseinschränkungen

Die Orthese darf nicht zur Prävention von Kontrakturen (Verkürzungen) usw. verwendet werden.

Die Orthese darf nicht bei sportlichen Aktivitäten mit Sprüngen, plötzlichen Bewegungen oder

schnellen Schrittfolgen verwendet werden (z.B. Basketball, Badminton, sportliches Reiten).

Sportliche Aktivitäten sollten generell mit dem Patienten besprochen werden.

3.6 Wirkungsweise

• Ermöglicht einen weitgehend physiologischen Gang, symmetrisch und flüssig

• Unterstützt die Kniestreckung und die Kniebeugung

• Unterstützt die Kniestreckung während der Zehenablösung

• Verhindert das unkontrollierte Aufsetzen und Absenken des Fußes während des Fersenauf

tritts

• Beeinflusst im Stehen die Statik in frontaler sowie sagittaler Ebene

5 Gebrauch

WARNUNG

nes Kraftfahrzeugs und lassen Sie die Fahrtüchtigkeit von einer autorisierten Stelle überprü

fen.

►Die tägliche Tragedauer und der Anwendungszeitraum werden in der Regel vom Arzt festge

legt.

►Die erstmalige Anpassung und Anwendung des Produkts darf nur durch Fachpersonal erfol

gen.

►Weisen Sie den Patienten in die Handhabung und Pflege des Produkts ein.

►Weisen Sie den Patienten darauf hin, dass er umgehend einen Arzt aufsuchen soll, wenn er

außergewöhnliche Veränderungen an sich feststellt (z.B. Zunahme der Beschwerden).

5.1 Auswählen der Größe

►Die Orthesengröße anhand der Schuhgröße auswählen (siehe Größentabelle).

5.2 Anpassen

VORSICHT

Unsachgemäßes Anformen oder Anlegen

Verletzungen und Beschädigungen an der Orthese durch Überbelastung des Materials und

falscher Sitz der Orthese durch Bruch tragender Teile

►Nehmen Sie keine unsachgemäßen Veränderungen an der Orthese vor.

►Legen Sie die Orthese immer gemäß den Anweisungen in der Anleitung an.

HINWEIS

Schäden durch unzulässige Handhabung (Wärmebehandlung, Bohren von Löchern)

Funktionsverlust der Orthese

►Führen Sie keine Wärmebehandlung an der Orthese durch. Die Orthese ist nicht thermo

plastisch verformbar.

►Bohren Sie keine Löcher in die Standfläche der Sohle oder der Verbindungselemente, da

Löcher die Fasern unterbrechen und das Passteil schwächen.

HINWEIS

Verwendung ohne geeigneten Schuh

Funktionseinschränkung durch unzureichende Stabilisierung

►Verwenden Sie die Orthese nur mit geschlossenem Schuh unter Berücksichtigung der zu

lässigen effektiven Absatzhöhe.

Wadengurt an der Unterschenkelschale anbringen

1) Beide Gurte öffnen und vom Klett abziehen (siehe Abb.2, siehe Abb.3).

2) Das Polster über die Flügel der Unterschenkelschale ziehen, dazu das Polster auf der Innen

seite und den Gurt auf der Außenseite der Schale platzieren (siehe Abb.4, siehe Abb.5).

3) Die Gurte straffen und am Klett am gegenüberliegenden Flügel der Unterschenkelschale be

festigen (siehe Abb.6, siehe Abb.7).

Schuhauswahl

►Einen stabilen Schnür- oder Klettschuh mit fester Fersenkappe wählen, um eine optimale Wir

kung der Orthese zu gewährleisten. Die effektive Absatzhöhe sollte zwischen 0,5cm und

1,5cm liegen.

Orthese anpassen

1) Falls der Patient einen Schuh mit herausnehmbarer Innensohle verwendet, die Innensohle

aus dem Schuh der zu versorgenden Seite entfernen.

INFORMATION: Die Innensohle aufbewahren, um den Schuh ohne die Orthese zu ver

wenden.

2) Den Patienten mit Schuh und ohne Orthese auf einen Stuhl setzen. Den Fuß im 90°Winkel

zum Unterschenkel positionieren (siehe Abb.8).

3) Die Position des Knöchelgelenks im Schuh mit einem kleinen Klebestreifen markieren (siehe

Abb.8).

4) Den Patienten ohne Schuh und mit Orthese auf einen Stuhl setzen. Den Fuß im 90°Winkel

zum Unterschenkel positionieren.

INFORMATION: Die Orthese so anlegen, dass der Fußknöchel genügend Bewegungs

freiheit hat und nicht an die Feder stößt.

5) Die Position des Fußknöchels bestimmen und auf dem Orthesenrahmen mit einem kleinen

Klebestreifen markieren (siehe Abb.9).

6) Die Orthesensohle so zuschneiden (siehe Abb.10), dass die Markierungen vom Schuh und

vom Orthesenrahmen übereinstimmen (siehe Abb.11).

7) Die Form der Sohle entsprechend der herausnehmbaren Innensohle anpassen. Dabei beach

ten, dass die benötigte Außenrotation des Fußes beibehalten wird (siehe Abb.10).

8) Die Orthese in den Schuh einsetzen und optional mit einer Einlage überdecken (siehe

Abb.12).

9) Die Orthese zusammen mit dem Schuh anziehen (siehe Abb.13).

10) Die korrekte a-p Position (a-anterior, p-posterior) des Kniegelenks messen (z.B. auf dem 3D

L.A.S.A.R Posture).

INFORMATION: Die Belastungslinie des Kniegelenks in a-p Position liegt ca. 15mm

vor dem Kompromissdrehpunkt nach Nietert.

11) Optional: Mit Hilfe von Keilen einen Ausgleich herstellen, um die gewünschte Position zu er

reichen: z.B. einen Ausgleich am Vorfuß platzieren (siehe Abb.17), falls der Patient zu stark

in Flexion steht oder einen Ausgleich an der Ferse platzieren (siehe Abb.18), falls der Patient

zu stark in Extension steht.

INFORMATION: Optional können auch andere, als die mitgelieferten Materialien zum

Aufbau der unteren Sohle verwendet werden.

12) Optional: Den obere Rand und die Flügel der vorderen Anlage beschleifen und die Polste

rung entsprechend ändern.

HINWEIS! Die Funktion der Orthese darf nicht beeinträchtigt werden. Nicht in den Be

reich der Feder oder die Oberflächen beschleifen!

13) Optional: Falls der Patient eine Fußdeformität aufweist, diese mit einer Einlage oder einem

speziell geformten Hilfsmittel korrigieren.

28U24, 28U25:INFORMATION: Als Zubehör bietet der Hersteller einen lateralen Prona

tionszug 28Z10, der an der lateralen Sohle angeklettet werden kann. Ein frontaler und

dorsaler Zug stabilisiert den Fuß mit Hilfe von Klett im Bereich der Feder.

14) Die Textilkomponente auf Wadenhöhe verschließen (siehe Abb.13).

Optional: Wadengurt kürzen

> Die Orthese dem Patienten anlegen.

1) Die Länge des Gurts ermitteln.

2) Den Y-Klett entfernen und den Gurt mit einer Schere kürzen.

3) Den Y-Klett am Gurt anbringen.

►VORSICHT! Der Y-Klett des Wadengurts der Kindergrößen kann von Kleinkindern ver

schluckt werden. Den Y-Klett nach dem Beenden der Anpassung mit den Wadengurt vernä

hen.

Sitz der Orthese prüfen

►Vor Abgabe der Orthese an den Patienten den Sitz der Orthese prüfen.

►Den Patienten zur Probe gehen lassen, unbedingt auch Schrägen, Rampen und Treppen tes

ten und bei Bedarf Feinanpassungen vornehmen.

5.3 Anlegen

VORSICHT

Falsches oder zu festes Anlegen

Lokale Druckerscheinungen und Einengungen an durchlaufenden Blutgefäßen und Nerven

durch falsches oder zu festes Anlegen

►Stellen Sie das korrekte Anlegen und den korrekten Sitz des Produkts sicher.

HINWEIS

Verwendung eines verschlissenen oder beschädigten Produkts

Eingeschränkte Wirkung

►Prüfen Sie das Produkt vor jedem Gebrauch auf Funktionstüchtigkeit, Verschleiß und Be

schädigungen.

►Verwenden Sie ein nicht mehr funktionstüchtiges, verschlissenes oder beschädigtes Pro

dukt nicht weiter.

1) Die Innensohle aus dem Schuh der zu versorgenden Seite entfernen.

2) Den Wadengurt öffnen.

3) Die Orthese in den Schuh stecken.

4) Die Orthese zusammen mit dem Schuh anziehen.

5) Den Wadengurt schließen.

5.4 Ablegen

1) Den Wadengurt öffnen.

2) Den Schuh mit der Orthese ausziehen.

3) Optional: Die Orthese aus dem Schuh entnehmen und die Innensohle des Schuhs wieder

einlegen.

6 Reinigung

HINWEIS

Verwendung falscher Reinigungsmittel

Beschädigung des Produktes durch falsche Reinigungsmittel

►Reinigen Sie das Produkt nur mit den zugelassenen Reinigungsmitteln.

►Ottobock empfiehlt die Orthese 2-mal wöchentlich zu reinigen.

Textilkomponente

►HINWEIS! Rückstände des Waschmittels können Hautreizungen und Materialver

schleiß verursachen.

1) Die Textilkomponente von der Orthese entfernen.

2) Alle Klettverschlüsse schließen.

3) Optional: Ein Wäschenetz verwenden.

4) Die Textilkomponente bei 30°C mit einem handelsüblichen Feinwaschmittel waschen.

Waschmittelreste sorgfältig ausspülen.

5) An der Luft trocknen lassen. Keine direkte Hitzeeinwirkung (z.B. Sonnenbestrahlung, Ofen-

oder Heizkörperhitze).

Orthesenrahmen

1) Bei Bedarf mit einem feuchten Lappen abwischen.

2) An der Luft trocknen lassen. Direkte Hitzeeinwirkung vermeiden (z.B. durch Sonnenbestrah

lung, Ofen- oder Heizkörperhitze).

7 Verpackung

Verpackung gelangt in die Hände von Kindern

These instructions for use provide important information on the fitting and application of the

28U24 WalkOn Reaction, 28U25 WalkOn Reaction plus, WalkOn Reaction Junior, 28U34

WalkOn Reaction Lateral and 28U34 WalkOn Reaction Lateral Junior ankle-foot orthoses.

2 Product overview

3 Intended use

3.1 Indications for use

The orthosis is intended exclusively for orthotic fittings of the lower limbs and exclusively for

contact with intact skin.

Ottobock recommends wearing the product with a stocking and not directly on the skin. A thin

stocking made of a washable, moisture-absorbent material is suitable to wear under the product.

To avoid pressure points, make sure that the stocking is free of wrinkles.

The orthosis must be used in accordance with the indications.

The doctor decides the daily duration of use for the product.

•28U25 WalkOn Reaction Junior: The orthosis may be combined with other dynamic

foot/ankle foot orthoses if the biomechanical requirements can be met more effectively as a

result.

3.2 Indications

• Minor impairment of plantarflexors

• Minor impairment of knee extensors

• Drop foot, with or without mild to moderate spasticity

Caused for example by:

• Stroke

• Traumatic brain injuries

• Multiple sclerosis

• Neuromuscular atrophy

• Peroneal nerve injury

• Infantile cerebral palsy (ICP)

• Impairment of the plantar flexor muscles

• Impairment of knee extension

Indications must be determined by the physician.

3.3 Contraindications

3.3.1 Absolute Contraindications

• Moderate to severe spasticity in the lower leg

• Moderate to severe oedema

• Moderate to severe foot deformities

• Leg ulcers

3.3.2 Relative Contraindications

The following indications require consultation with a physician: skin diseases/injuries, inflamma

tion, prominent scars that are swollen, reddening and hyperthermia of the fitted limb/body area;

lymphatic flow disorders, including unclear soft tissue swelling distal to the body area to which

the medical device will be applied; sensory and circulatory disorders in the lower leg and the foot

area.

3.4 Lifetime

When used as intended, the product is designed for:

Junior 1year (shoe sizes EU 24-27, 27-30, 30-33, 33-36)

Adults 2years (shoe sizes EU 36-39, 39-42, 42-45, 45-48)

When the end of the lifetime is reached, have the product inspected for functionality by qualified

personnel. Use of the product beyond the specified lifetime is the responsibility of the user.

3.5 Restrictions for use

The orthosis must not be used for the prevention of contractures (shortening) etc.

The orthosis must not be used for sports activities that involve jumping, sudden movements or

rapid step sequences (e.g. basketball, badminton, riding sports). Sports activities should gener

ally be discussed with the patient.

5 Use

WARNING

Operation of motor vehicles

and have your driving ability examined and certified by an authorised agency.

INFORMATION

►The daily duration of use and period of application are generally determined by the physi

cian.

►The initial fitting and application of the product must be carried out by qualified personnel.

►Instruct the patient in the handling and care of the product.

►Instruct the patient to see a physician immediately if any exceptional changes are noted (e.g.

worsening of the complaint).

5.1 Size Selection

►Select the orthosis size based on the shoe size (see sizing table).

5.2 Adaptation

orthosis due to breakage of load-bearing components

►Do not make any improper changes to the orthosis.

►Always apply the orthosis according to the information in the instructions.

►Do not carry out any heat treatment on the orthosis. The orthosis is not thermoformable.

►Do not drill any holes in the bottom of the sole or the connecting elements as holes break the

fibres and weaken the component.

Use without suitable footwear

Restriction of functionality due to insufficient stabilisation

►Only use the orthosis with closed footwear, taking the allowable effective heel height into

account.

Attaching the calf strap to the lower leg shell

1) Open both straps and detach them from the hook-and-loop (see fig.2, see fig.3).

2) Pull the padding over the wings of the lower leg shell, positioning the padding on the inside

and the strap on the outside of the shell (see fig.4, see fig.5).

3) Tighten the straps and attach them to the hook-and-loop on the opposite wing of the lower leg

shell (see fig.6, see fig.7).

Selecting footwear

►Choose a sturdy shoe with laces or hook-and-loop fasteners and a solid toe cap to ensure the

optimal effectiveness of the orthosis. The effective heel height should be between 0.5cm and

1.5cm.

Adapting the orthosis

1) If the patient uses footwear with removable insoles, take the insole out of the shoe on the fit

ting side.

INFORMATION: Keep the insole to use the shoe without the orthosis.

2) Have the patient sit on a chair with footwear and without the orthosis. Position the foot at a

90°angle to the lower leg (see fig.8).

3) Mark the position of the ankle joint in the shoe with a small piece of adhesive tape (see fig.8).

4) Have the patient sit on a chair without footwear and with the orthosis. Position the foot at a

90°angle to the lower leg.

INFORMATION: Apply the orthosis so the ankle bone has adequate freedom of move

ment and does not contact the spring.

5) Determine the position of the ankle bone and mark it on the orthosis frame with a small piece

of adhesive tape (see fig.9).

6) Trim the orthosis sole (see fig.10) so the markings on the shoe and the orthosis frame line up

(see fig.11).

7) Adapt the shape of the sole according to the removable insole. In doing so, make sure that

the required external rotation of the foot is maintained (see fig.10).

8) Insert the orthosis in the shoe and optionally cover it with an insole (see fig.12).

9) Put on the orthosis together with the shoe (see fig.13).

10) Measure the correct a-p (anterior-posterior) position of the knee joint (e.g.on the 3D

L.A.S.A.R Posture).

INFORMATION: The load line of the knee joint in the a-p position lies approx. 15mm

in front of the compromise knee pivot point according to Nietert.

11) Optional: Use wedges as compensating elements to achieve the desired position.

Forexample, place a compensating element under the forefoot (see fig.17) if the patient

stands with excessive flexion or place a compensating element under the heel (see fig.18) if

the patient stands with excessive extension.

INFORMATION: Optionally, materials other than those supplied may also be used to

build up the bottom sole.

12) Optional: Sand the upper edge and the wings of the frontal support and modify the padding

accordingly.

NOTICE! The functionality of the orthosis must not be impaired. Do not sand in the

area of the spring or the surfaces!

13) Optional: If the patient has a foot deformity, correct it with an insole or a custom moulded

device.

28U24, 28U25:INFORMATION: The manufacturer offers the 28Z10 lateral pronation

strap as an accessory that can be attached to the lateral sole with hook-and-loop

fasteners. Frontal and dorsal traction stabilises the foot using hook-and-loop fasten

ers in the area of the spring.

14) Close the textile component at calf height (see fig.13).

Optional: Shortening the calf strap

> Put the orthosis on the patient.

1) Determine the length of the strap.

2) Remove the Y hook-and-loop and shorten the strap using scissors.

3) Fasten the Y hook-and-loop to the strap.

►CAUTION! The Y hook-and-loop of the calf strap may be swallowed by young children.

Sew the Y-hook-and-loop to the calf strap after fitting.

Checking the fit of the orthosis

►Check that the orthosis is fitted properly before handing it over to the patient.

►Conduct trial walking with the patient. Testing on inclines, ramps and stairs is mandatory.

Fine tune as required.

5.3 Application

1) Remove the insole from the shoe on the side to be fitted.

2) Open the calf strap.

3) Insert the orthosis into the shoe.

4) Put the orthosis on together with the shoe.

5) Fasten the calf strap.

5.4 Removal

1) Open the calf strap.

2) Remove the shoe together with the orthosis.

3) Optional: Remove the orthosis from the shoe and re-insert the insole of the shoe.

6 Cleaning

►Ottobock recommends cleaning the orthosis twice a week.

Textile component

►NOTICE! Detergent residues may cause skin irritation and wear of material.

1) Remove the textile component from the orthosis.

2) Fasten all hook-and-loop closures.

3) Optional: Use a mesh laundry bag.

4) Wash the textile component in warm water at 30°C with a standard mild detergent. Rinse out

detergent residues thoroughly.

5) Allow to air dry. Do not expose to direct heat (e.g.from the sun, an oven or radiator).

Orthosis frame

1) Wipe with a damp cloth as needed.

2) Allow to air dry. Do not expose to direct heat sources (e.g. sunlight, stove or radiator).

7 Packaging

The product is delivered in a carrier bag. The carrier bag is intended exclusively for transporting

3.1 Usage prévu

L’orthèse est exclusivement destinée à un appareillage orthétique de l’extrémité inférieure et elle

est conçue uniquement pour entrer en contact avec une peau intacte.

Ottobock recommande de ne pas porter le produit directement sur la peau mais après avoir enfilé

un bas. Utilisez pour ce faire un bas fin fabriqué dans une matière lavable et absorbant l’humidité.

Veillez à ce que le bas soit mis en place sans plis, sans quoi il pourrait être à l’origine de pres

sions.

Il est impératif d’utiliser l’orthèse conformément aux indications.

Le médecin décide de la durée de port du produit.

•WalkOn Reaction Junior 28U25: la combinaison de l’orthèse avec un autre pied dyna

mique/d’autres releveurs de pied est autorisée si elle permet de mieux répondre aux exi

gences biomécaniques.

3.2 Indications

3.3 Contre-indications

3.3.1 Contre-indications absolues

• Œdème modéré à sévère

• Déformations du pied modérées à sévères

• Ulcères de jambe

3.3.2 Contre-indications relatives

Les indications suivantes requièrent la consultation d’un médecin: lésions ou affections cuta

nées, inflammations, cicatrices exubérantes avec œdème, rougeurs et échauffement excessif

dans la zone du corps appareillée, troubles de la circulation lymphatique et également œdèmes

inexpliqués des parties molles dans la partie distale de l’orthèse, troubles de la perception et de

la circulation sanguine du bas de la jambe et du pied.

3.4 Durée de vie

Dans la mesure où l’utilisation est conforme, le produit est conçu pour:

Junior 1an (pointures UE: 24-27, 27-30, 30-33, 33-36)

Adultes 2ans (pointures UE: 36-39, 39-42, 42-45, 45-48)

À expiration de la durée de vie, faire contrôler le produit par un personnel spécialisé qui

s’assurera de sa fonctionnalité. Tout usage au-delà de la durée de vie indiquée se fait sous la

seule et entière responsabilité de l’utilisateur.

3.5 Restrictions d’utilisation

L'utilisation de l'orthèse n'est pas autorisée pour prévenir les contractures (raccourcissements),

etc.

L'utilisation de l'orthèse n'est pas autorisée pour pratiquer des activités sportives au cours des

quelles des sauts, des mouvements soudains ou des cadences rapides de pas sont effectués

(par ex. le basketball, le badminton ou l'équitation sportive). Il est conseillé d'aborder le thème

des activités sportives avec le patient.
